Shift Performance Lab — Practice Waiver & Consent

Practice sessions: scheduled practice sessions at Shift Performance Lab private training facility. The full facility address is provided to registered families separately and is not published on this site.

1. Consent to participate. The parent/guardian consents to the player participating in youth soccer development sessions (1v1, 2v2, especially 3v3).

2. Acknowledgment of risk. Soccer and outdoor training carry injury risks, including heat, rain, lightning and severe weather, insects, and uneven or wet surfaces. Evening sessions may be after dark under facility lighting. Pool and other private property areas are off-limits during sessions unless the coach expressly states otherwise, and players are not supervised in those areas.

3. Release of liability. To the fullest extent allowed by law, the parent/guardian releases Shift Performance Lab, its coach/director, staff, volunteers and the facility property owner, except for negligence or intentional misconduct, and accepts financial responsibility for damage the child causes.

4. Medical authorization. Staff may provide basic first aid and, in an emergency, call 911 and authorize emergency treatment/transport if the parent cannot be reached. The parent is responsible for medical costs.

5. Supervision, drop-off and pick-up. Players are supervised from check-in until release to a parent or authorized person. The parent is responsible before check-in and after release. Players must stay in designated training and parent areas.

6. Photo / video. Sessions may be recorded by facility cameras. The program provides a video report once a month. Separate consents below cover (a) appearing in session recordings used for that player's own video report, and (b) photo/video use beyond the family's own report.

7. Agreement. By signing in Step 3, the parent/guardian agrees to this waiver.

This on-site display is generalized (no specific practice dates/times). The underlying waiver document remains subject to attorney/insurer review before use — see README.

How do jersey numbers work?
Pick from the numbers currently available. A number is claimed per player at registration.

What if my player is already in another club?
Enter the current club at registration, or “N/A” if not affiliated.

Do tournaments need a separate waiver?
Yes — tournaments may require a separate organizer waiver, sent separately when the team enters a tournament.

Is checkout live?
No. Checkout stays disabled until the EIN, bank account and Stripe are set up.
